Atopic dermatitis endotypes based on allergen sensitization, reactivity to Staphylococcus aureus antigens, and underlying systemic inflammation.

A new study published in The Journal of Allergy and Clinical Immunology. In Practice aimed to evaluate allergy-driven endotypic differences in atopic dermatitis (AD) patients and identify proteomic signatures to distinguish between inflammatory responses of AD. This study entailed the collection of sera from 76 subjects with moderate-to-severe AD and 39 healthy controls (HCs) with no history of skin disease. Serum was tested for levels of total serum IgE and allergen-specific IgE using a panel of 119 allergens, as well as IgE antibodies against Staphylococcus aureusantigens, and was profiled for more than 1100 proteins, to detect differential expression of inflammatory mediators. It was observed that total serum IgE levels were significantly elevated in subjects with AD relative to controls. A greater percentage of AD subjects were allergic compared to the HCs, and AD patients tested positive to a greater number of allergens than HCs. Meanwhile, IgE was upregulated across four allergen subsets – food, perennial, seasonal and mixed; each allergen subset was associated with a distinct inflammatory signature marked by a specific suite of upregulated proteins. Furthermore, IgE antibodies against S. aureus TSST-1 toxin were significantly upregulated in seasonal allergy and perennial allergy subjects. In inference, it was stated that this study addressed the heterogeneity of AD by characterizing subsets of AD based on allergen sensitization and also demonstrated the differential systemic inflammation and S. aureus-specific antibody responses associated with the allergenic endotypes. These unique proteomic signatures may be valuable for disease characterization and subsequent personalized treatment.
